The Heart Failure Clinic at Premier Heart and Vascular Center provides service to our patients 24-72 hours post hospital discharge when treated for an acute heart failure exacerbation. We also provide emergent intravenous diuretic therapy in office on a case by case basis. Our goal is to help optimize our patients with guideline directed medical therapy to help prevent hospital re-admission by educating our patients and their family on the necessary lifestyle changes.
Our clinic is managed by a licensed RN. We also provide CHF monitoring online for patients that qualify.
